By requesting ICEERS’ services you authorize the practitioner providing your care to share information with other practitioners in the following circumstances:
- If there is sufficient reason to believe that an adult is required to ensure protection (relating to abuse, neglect, exploitation or abandonment), your data may be transferred to the local agency providing the social services that the situation requires.
- If you manifest a serious threat, or intent to kill or seriously injure a person or group of persons who can be personally identified, and if there is sufficient reason to believe that you may carry out such a threat, reasonable steps will be taken to prevent any harm. These reasonable measures may include notifying the potential victim of the threat.
- If there is sufficient reason, based on the professional judgment of the service team, to suspect that a child abuse situation exists.
